•	Decree Absolute if either ot-party is divorced. Court stamp is required.
•	Chanqe of name by Deed Poll, proof is required. including Solicitor sianature and stamp.
•	Adoption Certificates if applicable
•	Death Certificates if any of the party is a Widow/Widower
	Parental Consent if under 18 years old.
•	Wedding form completed... Stating names. Address, occupations and place of employment for both parties.
